We provide a broad spectrum of traditional and full-service partnership programs designed to assist adults and older adults living with mental health and substance abuse challenges in regaining hope, developing self-determination and taking personal responsibility.   • Dual Diagnosis
    • Who

      Adults ages 18 and older who are experiencing mental health challenges and also have substance use challenges.

  • Full Service Partnership Continuum Adult
    • Who

      Adults ages 26 and older who meet one (1) of the following criteria:

      • 2-4 inpatient hospitalizations in the last 12 months
      • 2-8 emergency psychiatric admissions in the last 12 months
      • 120 days of chronic homelessness or being unhoused
      • patterns of service that rely almost exclusively on emergency and/or institutional care

      • Increase stable living arrangements and reduce chronic homelessness
      • Reduce hospitalizations, both inpatient and emergency psychiatric admissions
      • Increase self-sufficiency and independence

      • “Whatever it takes” recovery model

  • Traditional Adult
    • Who

      Adults ages 18 and older who meet both of the following criteria:

      1. Have a diagnosed mental health disorder or suspected mental health disorder not yet diagnosed
      2. Have significant impairment, defined as distress, disability, or dysfunction in social, occupational or other important activity

      • Decrease impairment in significant activity
      • Increase natural support networks

  • AB109 Reentry Services
    • Who

      Adults ages 18-59 who are experiencing mental health or co-occurring mental health and substance use challenges and have had multiple incarcerations and involvement in the criminal justice system.

    • Program Goals

      • Provide culturally-competent, strengths-based services for wellness and recovery
      • Promote mental and physical well being
      • Reduce aggressive and/or criminal behavior
      • Decrease impact of trauma and increase resiliency
      • Support high levels of personal accountability and personal/civic achievement

      • Case management
      • Counseling (individual, group and/or family)
      • Substance use treatment
      • Medication support services
      • Crisis intervention and after-hours crisis line

  • Aftercare Services
    • Who

      Adults ages 18 and older who are current Department 61 clients and who have a history of mental health or substance use challenges and are recently off probation from the criminal justice system or ending probation soon. Individuals must be able to manage their own medications with limited assistance.

    • Program Goals

      • Promote healthy thinking and behavior patterns to support transition away from criminal justice system
      • Provide culturally-competent, strengths-based services for wellness and recovery
      • Promote mental and physical well being
      • Decrease impact of trauma and increase resiliency

    • Services

      • Weekly support groups
      • Assistance identifying community resources to support recovery
      • Access to a psychiatrist for medication management
      • Case management (as needed)
      • Therapy (as needed)

  • Criminal Justice Full Service Partnership (FSP)
    • Who

      Adults ages 18-59 who are experiencing mental health challenges and have had multiple incarcerations and involvement in the criminal justice system.

  • Co-Occurring
    • Who

      Adults ages 18-59 involved in the criminal justice system who are experiencing mental health challenges and substance use challenges.

  • Forensic Assertive Community Treatment (FACT)
    • Who

      Adults ages 18+ who have had a difficult time engaging in treatment due to varying degrees of involvement with the criminal justice system and experience with co-occurring mental health, substance use and/or physical health disorders that are often under-managed. This program also serves a small number of individuals who have been deemed incompetent to stand trial, with the purpose of restoring competency.

    • Program Goals

      Through extensive outreach and continuous engagement, this program works to improve the individual’s insight and progress toward achieving their goals. Program goals include:

      • Provide strengths-based recovery, while promoting mental and physical wellness
      • Improve client’s mental health outcomes and daily functioning
      • Achieve reductions in recidivism by addressing criminogenic risks and needs
      • Support high levels of personal accountability
      • Support high levels of personal and civic achievements
      • Restore competency (for applicable clients)

  • Jail Diversion
    • Who

      Adults ages 18 and over with severe mental illness and/or substance use disorder(s) who may be homeless and who are involved in the criminal justice system.

    • Program Goals

      • Get individuals into housing immediately
      • Provide culturally-competent, strengths-based services for wellness and recovery from mental health and/or substance use challenges
      • Support individuals through the criminal justice system
      • Decrease impact of trauma and increase resiliency

    • Services

      • Intensive case management
      • Life skills and psychoeducation
      • Counseling (individual, group and/or family)
      • Medication support services
      • Substance use treatment
      • Permanent Housing
      • Crisis intervention and after-hours crisis line

  • Intensive Case Management (ICM)
    • Who

      Adults ages 18 and older who are chronically homeless

    • Program Goals

      • Assist chronically homeless adults in obtaining and retaining permanent housing,
      • Provide services to support independent living and promote housing stability
      • Provide culturally-competent, strengths-based services for wellness and recovery from mental health and/or substance use challenges
      • Ensure access to appropriate health services
      • Support individuals through the criminal justice system
      • Decrease impact of trauma and increase resiliency

      • Intensive case management
      • Life skills and psychoeducation
      • Counseling (individual, group and/or family)
      • Medication support services
      • Substance use treatment
      • Permanent housing
      • Crisis intervention and after-hours crisis line

  • La Casa del Puente Transitional Residential
    • Who

      Adults ages 18-59 who are transitioning from a locked mental health facility who need to stabilize their medications, wellness, recovery and daily living skills.

      • Community-based residential treatment, 30 days – 90 days
      • Case management
      • Counseling (individual, group and/or family)
      • Medication support services
      • Crisis intervention
      • Advocacy

  • Madrone Crisis Residential
    • Who

      Adults ages 18-59 who have experienced an emotional and psychiatric episode and who have not benefited from traditional outpatient mental health services, primarily those coming from an acute hospital setting who can benefit from short-term residential stabilization.

    • Program Goals

      • Provide timely access to community and culturally based behavioral health crisis services
      • Provide culturally-competent, strengths-based services for wellness and recovery
      • Develop safety, interpersonal and independent livings skills, and community support systems
      • Decrease impact of trauma and increase resiliency
      • Increase access for family members and other social support systems to be immediately and continually involved in the treatment process
      • Successfully reintegrate individuals back into the community

      • 21-day residential program
      • Immediate medical and psychiatric assessment

      Emergency Psychiatric Services (EPS), hospitals and law enforcement. All referrals must be approved and verified through Santa Clara County 24-Hour Care.

    • Who

      This program is designed to meet the complex needs of individuals and families experiencing chronic homelessness by providing wraparound services that will help them foster independence.

  • Transitional Housing Units (THUs)
    • Who

      Adults ages 18-59 who are enrolled in Community Solutions’ Criminal Justice Full Service Partnership program and are involved in the criminal justice system.

    • Program Goals

      • Provide a clean and sober living environment
      • Provide culturally-competent, strengths-based services for wellness and recovery
      • Decrease impact of trauma and increase resiliency
      • Support high levels of personal and civic achievement

    • Services

      • Supportive housing
      • Weekly house meetings
      • Crisis intervention and after-hours crisis line
      • Referral and linkage to additional services and resources

    • Referrals

      Clients participating in Community Solutions’ Criminal Justice Full Service Partnership program are eligible for this program. Referrals may also be accepted through other service providers.

    • Fees/Insurance

      To participate in this program, residents pay 30% of GA/SSI.
